R5-D4

R5-D4 was an R5 series astromech droid that was built by Industrial Automaton for the Galactic Empire. He was assigned to serve on a Star Destroyer, but during a routine inspection, it was discovered that he had a defective motivator. This caused him to malfunction and fail his mission, resulting in his being decommissioned and sold into the black market.
